Our work

The Health Alliance works closely with community and key stakeholders across the North Brisbane and Moreton Bay region to identify barriers in giving and receiving health care. Based on this engagement, key focus areas are developed to prioritise which initiatives are delivered.

The current priorities of the Alliance include:

Trialling the provision of a shared care model of Hospital in the Home for First Nations clients to improve the management and care of diseases and/or illnesses that are the leading contributors to the health gap experienced by Indigenous peoples.

Designing a collective impact approach to overcome fragmentation and improve health outcomes for children and families in the Caboolture region.

Establishing a co-commissioning model to enable better outcomes from mental health funding provided to the region.

Trialling a proactive care coordination and case management model to improve access to care closer to home and reduce demand on Emergency Departments (EDs).
